Mode of Baptism as Represented: Post-Apostolic Writings to Reformation

I am looking for standard or definitive resources that examine the mode of baptism as represented throughout the history of the church from post-apostolic times to the Reformation. Thank you in advance.
 
The standard academic work for the first five centuries is Everett Ferguson’s Baptism in the Early Church. It is very comprehensive, making it quite lengthy (almost 1000 pages) and a bit pricey. It covers a lot more than mode - historical backgrounds, word usage, theology, liturgy, credo/paedo issues, etc. - and primary source references abound. It has copious footnotes and extensive indexing, so you can easily pick out specific topics like mode to read on. Also available to borrow at Archive. [Edit: it looks like Archive has changed the availability of the book to only those with "print disabilities."]

I'm not aware of an exhaustive historical documentation of medieval practice. A lengthy thread I did on early Protestant practice includes some looking back at the period leading up to the Reformation.

The Didache

7. 1 Regarding baptism. Baptize as follows: after first explaining all these points, baptize in the name of the Father and of the Son and of the Holy Spirit (Mt 28:19), in running water. 2 But if you have no running water, baptize in other water; and if you cannot in cold, then in warm. 3 But if you have neither, pour water on the head three times in the name of the Father and of the Son and of the Holy Spirit (Mt 28:19). 4 Before the baptism, let the baptizer and the candidate for baptism fast, as well as any others that are able. Require the candidate to fast one or two days previously.
